The documentation will be emailed to the client before the session and after the initial consultation, asking for date of birth, address, beliefs, medication, and health condition. 

The client will need to sign off the Consent and Agreement:
I understand that hypnotherapy is a complementary therapy and is not a substitute for medical or mental health treatment. I agree to participate in the hypnotherapy session(s) voluntarily and acknowledge that the therapist cannot guarantee specific outcomes.
